The Clinical Ritual of Evaluating a Submissive Male

The Complete Sub Physical Exam
At FemdomU, health is not separate from obedience. It is one of its cornerstones. A submissive’s body must be maintained, evaluated, and trained like any other tool in service to a Domme. That philosophy carries over from how Madam Nora considers taking on a new personal submissive; she doesn’t just ask about his skills or his willingness. She brings me in to examine him.

Every new submissive undergoes a Complete Sub Physical Exam before Madam Nora accepts him for any form of service. I don’t do this in private. The exam is not meant to be gentle, nor modest. It is invasive, explicit, and intentionally exposing. That’s part of the point. If a boy cannot withstand being stripped, measured, and examined while on display, then he isn’t ready to serve a woman like Madam Nora.

Likewise – when Madam Nora has trained a sub and is releasing him to another’s ownership, we’ll conduct this thorough examination, typically while his new owner observes. This is just part of Nora’s high level of standards.

Let me walk you through the process, and explain what we check, and why.

Arrival and Stripdown

He is told to arrive nude, or to strip immediately in front of us. I typically have at least one or two other ladies assist, typically from Nora’s staff or perhaps one of her Dommes-in-training. I conduct the exam while other submissives observe. He is given no curtain, no gown, no modesty.

This first moment tells us a lot. How easily does he follow the order? Does he hesitate? Does he try to shield himself? Does he obey quickly, or performatively? His attitude at this stage is as important as what we see between his legs.

Posture, Inspection, and Measurements

I start with a visual full-body inspection. He stands in place, arms down, feet apart, while I walk around him slowly. I observe his posture, cleanliness, grooming, skin health, and muscle tone. Every inch is fair game, from scalp to soles.

Then come the measurements. I will typically have one of the other Dommes measure first, a perfect training moment. I’ll then repeat the measurements, and one of the other subs (this job typically falls to duckie) will be recording them as I confirm the following:

• Height and weight
• Waist, hips, chest, thighs, and biceps
• Cock length (flaccid and erect), and girth
• Scrotum drop and tightness – more of a subjective observation
• Standing and kneeling reach

These aren’t just numbers. They tell us how well he takes care of himself, whether he can present properly, and how his physical assets may be best used in service or display. Yes, his cock is measured. Yes, his balls are handled. This is not optional. My hands are gloved, and they will examine every inch of the property before Madam Nora takes possession.

Clinical Olfactory Test – How Does He Smell

I also include a clinical olfactory test. While not my favorite part, it is an important one. I check his scent at the armpits, groin, and anus. Fingers to skin, sniffed directly. I need to know if he maintains his hygiene, or if he hides decay beneath shaving and perfume. Some Dommes care deeply about this, and Madam Nora is highly sensitive to scent, so she demands perfection in this area.

(As a side-note, if any of you ever are close to duckie, take the opportunity to smell him. He has got to be the sweetest smelling boy I’ve ever known – like an ocean wave of fresh air and a tiny hint of strawberry.)

Cardiovascular and Mobility Testing

The potential sub is instructed to perform a treadmill test. I measure heart rate at rest and post-exertion, record pulse recovery, and log respiration. A basic fitness baseline is mandatory.

Then, I evaluate his range of motion. He is instructed to assume several submissive poses:

• Kneeling with hands behind back
• Forehead to floor with ass raised
• Push-up hold
• Spread-eagle on command
• Standing, hands behind head, eyes down

I look for shakiness, resistance, fidgeting, or poor flexibility. He is corrected when needed. This is also a subtle obedience test: does he hold position until released? Does he whine? Does he anticipate orders or wait properly?

Genital Function and Use Assessment

Next, I test his arousal. With gloved hands, I examine his penis and scrotum clinically, checking for abnormalities, sensitivity, blood flow, and erection quality. I may use lubricant, vibration, or simple stimulation to test his ability to respond. He is expected to get hard on command.

All boys are given a plug during this step. Subs have nothing of their own at this point, everything belongs to Madam. We provide a standard plug for the exam, and I check how comfortably and securely it fits. I observe muscle control, clenching, and ability to retain the plug under instruction.

Every sub is also caged. I fit his first device myself, ensuring it is properly sized and secure. I then remove it and after confirming proper fit, it is sent for engraving.

Rectal and Anal Health Exam

This part is always conducted in a presentation position. Most often, I have him kneel, chest down, ass raised, either on a padded bench or the floor.

I begin with a gloved, lubricated anal inspection. I test tightness, responsiveness, hygiene, and general health. Prostate palpation is done during this step, sometimes followed by controlled stimulation to assess his sensitivity and potential for anal orgasm. I take note of any hesitation or resistance.

As mentioned, all boys are given a plug. This is not decorative. It is to test readiness, compliance, and anatomical adaptability. If he is resistant as the plug is inserted and removed, such reactions are noted.

Laboratory Testing and Sample Collection

Every sub submits to a blood draw. I take a panel that includes hormone levels, STI screening, metabolic health, and basic function indicators. I collect a urine sample. When he ejaculates later in the exam, I preserve semen for lab analysis.

These are not optional. Madam Nora expects documented results.

Obedience Reflexes and Pain Response

I finish with a light corporal check. This is not punishment, but a diagnostic stroke or two from a flogger, paddle, or bare hand. It allows me to watch how his body reacts to controlled pain. Some flinch. Some shiver. Some get hard.

Dommes need to know which they’re dealing with.

Orgasm Testing and Release Evaluation

At the conclusion of the physical exam, every sub is required to demonstrate orgasm function and control. I handle this part myself to ensure a consistent clinical standard. The sub is stroked methodically, with a gloved hand, lubricant, and a set pace, until he reaches climax. I do not rush him. I maintain a steady rhythm, observing how quickly he builds, how he breathes, how his body tenses, and what sounds he makes.

I evaluate his orgasm for:

• Latency from start to ejaculation
• Volume and force of release
• Muscular contractions
• Emotional or verbal responses
• Post-orgasmic behavior

I note whether he begs, resists, performs theatrically, or submits quietly. If he dribbles, spurts, or erupts, each response provides insight into his arousal pathways and self-control.

His semen is immediately collected in a sterile specimen jar and labeled for lab analysis. This sample is analyzed for sperm count, hormone markers, and fluid quality. It is one of the most intimate data points we gather, and it is required.

A submissive who cannot perform this final task is not ready. A submissive who does, and does so well, earns his first moment of release in service.

Final Notes and Submission Protocol

After the exam, he is told to remain in position until dismissed. I often leave him kneeling while I consult with Madam Nora. That silence-naked, used, inspected, and forgotten for a moment, is part of his training, too.

My final report includes all measurements, physical reactions, lab results, notes on obedience and display, medical concerns, and a recommendation. I may advise further testing, short-term training, or immediate rejection. If he passes, he is accepted. If not, we send him back with a list of what needs correcting.
